- Important Disclosure: Hope Neurology is a Collaborative Practice. You may be seen by an assistant physician and have the right to see the collaborating physician.

✅️ Gather All Pertinent Records
- It is recommended to bring or email a timeline of your symptoms, any clinical notes by doctors or health professionals you have previously seen and any results for labs or testing so we can better understand how to help. You can email these to the HIPAA secure Medical Team email below if needed.
- It is strongly recommended that you provide any imaging studies (in addition to reports) and/or EEG studies via disk or flash drive, or simply request the study facility to share the study in a Shared Drive folder with our Medical Team at medicalteam@hopeneurology.net
