Personalized Treatment Plans

Hope Neurology offers personalized care through in person, virtual telehealth and at-home concierge services—bringing expert neurologists to you without the need to travel. As a fee-for-service practice, patients pay at the time of service.


Before your first visit, check with your insurance about out-of-network benefits as coverage varies. Most services are considered covered benefits under some plans. We also submit claims directly to insurers for a $10 fee or free with the concierge membership, helping to make your care affordable and seamless.

Appointment service fees are due at the time of scheduling and are refundable only if the patient cancels with at least 48 hours' notice. Additional charges may apply for additional in office treatments or procedures.

Additional Notes

  • Medicare patients require an Advance Beneficiary Notice (ABN) for non-coverage.
  • Tricare patients must sign an opt-out agreement.
  • Providers with Medicare relationships are legally prohibited from charging less than the Medicare limiting charge; in rare cases, this may increase the hourly rate.
  • Labs, imaging, medications (oral and IV), diagnostic testing, and referrals can be coordinated with your in-network provider at no extra charge to help keep your care affordable.

Why Have a Membership?

We offer patients a VIP specialty concierge monthly membership which offers several benefits such as: 

  • Lower fees for all services
  • Longer appointment times, allowing Providers to address your concerns in great detail
  • Cover basic primary care services, including annual workup labs, medication renewals, etc.
  • Free claim submission on your behalf to insurance
  • No cancelation fees
  • Priority booking
  • No fees for forms requested to be filled

All monthly memberships fees are Cash Pay service ONLY, and will not be submitted to insurance.  However, we do accept HSA for payment.

FAQ

Billing and Membership

"Is Dr. Dyer accepting new patients?"

Dr. Dyer accepts patients on a case by case basis into her concierge membership practice. Please note spaces are limited for these services.  Our team will be happy to place you on our waiting list if slots are unavailable.


“How does billing work with Hope Neurology?” 

Hope Neurology Is a fee-for-service practice, and patients pay at the time of service. In our provider fees we only charge you for time with us and for requested services we provide. If you need any imaging, labs, medications, and any diagnostic testing (including referrals) we can arrange with in-network care providers and in-network testing facilities for no cost to you to make your care as affordable as possible.


“How do I sign up for a Concierge Membership” 

During your first visit, you will be provided paperwork that consists of the Membership Services and Benefits as well as a membership agreement form.


“How do I cancel my Hope Neurology concierge membership” 

Membership requires a 3-month minimum. To cancel your membership after the three months, it requires a 30-day notice. Reach out to billing@hopeneurology.net when you want to give that 30-day notice. Cancellation of membership DOES NOT discharge you as a patient. Any services rendered after cancellation of membership will be charged at non-membership pricing.

Insurance

“Does Hope Neurology submit my claims to insurance” 

We submit the claim on your behalf and reimburse you accordingly. 


“I have myself or my child down as self-pay on the intake forms we filled out, why do you need an insurance card for us” 

If you have insurance but are paying through self-pay, we will still need your insurance card, so we are able to submit prior authorization for testing/imaging/medication as well as submitting your insurance information with lab work, to help make your care more affordable.


“My insurance company says that you are "in network", why am I still being provided with Hope Neurology’s provider fee schedule?”

We are currently "in network" with United Healthcare. Please note, for patients with United Healthcare, the concierge services including memberships with associated pricing are boutique clinic services versus those covered by your insurance. UHC pricing for services rendered provided by your carrier cover select services as agreed upon. The Boutique clinic services are different than what UHC offers, and the associated fees are also different and may not be covered by UHC.  Please check with your individual plan type to request what services are covered.
